Since disposable nursing pads threatened to break my wallet and adhesives were irritating, I swopped to cloth ones a long time ago. Just as my 3 pairs of cloth pads are getting worn out from hundreds of washes, I received a pair of Angel Mel Hot Mama Organic Nursing Pads from baby-mel.com to review.

About Angel Mel Organic Nursing Pads

These are the main features of the breast pads:

Top layer (next to skin): Bamboo velour (natural, undyed cream colour)

Inner core: Organic hemp

Outermost layer: 100% merino wool

Country of origin: Made in Malaysia

The pads are available in a range of prints designed by Robert Kaufman.

First Impressions

Oh my, these nursing pads are: Lovely. Super-soft. Super-smooth. I’ve a soft spot for bamboo velour and prints, and these nursing pads hit both right on!

How I Tested Them

Laundry: After every shower, I simply handwash the pads in cold water, wring and hang dry. After 1.5 months, they still remain soft and smooth.

Comfort: I have eczema and was initially concerned about the merino wool on the outside. The wool did nothing to my skin, since it faced outside anyway. The pads felt very comfortable and cool, which is fantastic for hot and humid weather. A concern is that the pointed tip of the nursing pads could show against thin bras and clothes. It’s better to use them with T-shirt bras.

Effectiveness: I’ve never been “leaky” (in between breastfeeding sessions) and already started weaning Vee when testing the nursing pads. So I experimented by pouring water slowly on them. They absorb A LOT!

The price of a pair of nursing pads is RM38.90 (~USD12 or SGD16). Since the materials are organic, very comfortable and come in prints, the price is considered reasonable. They’d still be much cheaper than disposable nursing pads, anytime.

—

What I like about it:

Fabric remains soft and smooth even after 1+ month of regular handwashing

Absorbent

Available in cute prints

What can be improved:

Update design to remove the pointed tip

—

To celebrate World Breastfeeding Week (1 to 7 August 2010), here are my top 7 reasons for loving breastfeeding, from a Stay-At-Home-Mum’s point of view:

1. Healthy for Baby

Breast milk is the perfect food designed for baby — loaded with antibodies and the composition changes naturally to fit baby’s nutritional needs. Vee has been a relatively healthy child since a newborn and I believe it is from the goodness of breast milk. (Read The Many Benefits of Breastfeeding at kellymom.com.)

2. Healthy for Mummy

Did you know that breastfeeding reduces the mother’s risk of breast, uterine and ovarian cancer? Breastfeeding is also a fast and easy way to lose all those pregnancy bulk. I cared for Vee full-time, dedicated little time for exercise, ate DOUBLE servings of food and stayed as slim as before. (Read 7 Ways Breastfeeding Benefits Mothers at AskDrSears.com.)

3. Free!

The best things in life are free? Yes, when it comes to breastfeeding. Inferred from the table in Which Milk is Better? (Part 2), we saved at least RM3500 (RM250 per month x 14 months) (~USD1100). The savings could be more since a younger baby would drink more milk than a toddler. Oh, and more savings from not buying more milk bottles and teats.

4. Best Baby Pacifier

When baby is crying, colicky and fussy, I have the best weapon as a last resort to regain peace — just pop him to the boobs. Initially I did not intend to co-sleep with Vee and nurse him through the night. But when he woke up every 45 to 60 minutes looking for me, and I lost sleep for a week, I GAVE UP. Suckle all you want and let us sleep, baby. I don’t know of any 20-year old who still breastfeeds, so you would definitely be weaned off one day.

5. Convenient

Breast milk is sterile and available for baby when he needs it. During travels, it is super-convenient, without having to bring milk powder, bottles and steriliser. There is also no concern about getting clean water to make baby’s milk. And no scare about milk tainted with melamine.

6. No Menses and No Cramps

My last menses was on 26 May 2008, 2 years and 2 months ago, until I got pregnant with Vee. I used to have severe menstrual cramps that led to vomiting, fainting and diarrhoea on the first day. It was such a phobia every month when “that day” was arriving. Thanks to Vee who does not sleep through the night, I have enjoyed many carefree months. (I read from a forum that one Mummy practised tandem breastfeeding for 3 children and had no menses for TEN years! I shall try to beat her record.)

7. Environmentally-Friendly

This occurred to me when 2handedoctopus (previous owner of Mia Bambina) pointed out in her Earth Day post. No production of tins, plastic covers, plastic scoops, plastic bottles and teats. No use of electricity to sterilise feeding equipment. Nature meant it to be this way.

Vee is starting to sleep though 5 to 6 hours, suckling only twice at night. I am sooooo going to miss breastfeeding him…

—

Sharing Time: If you are a breastfeeding Mum, what do you love most about it?

Jaundice is very common among newborns. During my prenatal class, the lactation consultant has prepared me for handling breastfeeding and jaundice. I was glad to have read “Jaundice in the Breastfed Baby” by Anne Smith, IBCLC. The article explains the various types of jaundice and supports the case for continued breastfeeding. I love her conclusion:

In most cases, jaundice is a normal, possibly even beneficial process that can be managed without interrupting breastfeeding. The treatment for physiologic jaundice is more breastfeeding rather than less, and sick babies with pathologic jaundice need breastmilk even more than healthy babies. Even in rare cases where the jaundice is caused by the breastfeeding, there is no reason to wean and every reason to continue giving your baby the best possible nourishment – mother’s milk.

A renowned doctor, Dr. Jack Newman, states the following in Breastfeeding and Jaundice:

There is not one bit of evidence that this [so-called breastmilk] jaundice causes any problem at all for the baby.

Do not stop breastfeeding for “breastmilk” jaundice.

When breastfed newborns get jaundice for a more prolonged period (past 2 to 3 weeks), some paediatricians start to point their fingers at breastfeeding as the cause, citing the so-called breastmilk jaundice. While these doctors know how to ask mums to stop breastfeeding temporarily, hardly any advice is provided on how to maintain the milk supply or prevent nipple confusion. In the end, babies could lose their once in a lifetime chance to be breastfed exclusively up to the period the mum originally intended.

Examples

CASE #1: Some months back, a friend’s baby’s paediatrician said that her newborn had breastmilk jaundice and was advised to stop breastfeeding and use formula-milk feeding for one week. She was not advised to express milk round the clock, so she expressed only when her breasts are full. Within a few days, her milk supply reduced drastically and was difficult to raise. She gave up breastfeeding. Original intention: breastfeed up to 6 months or more.

My thoughts: I deduce that the paediatrician is not pro-breastfeeding; instead likely a full supporter of formula milk. How do I know? I brought Vee to his clinic once, went to its storage area to breastfeed him and saw this HUGE shelf filled with tins and packets of formula milk! For a moment, I thought I was at a supermarket!

CASE #2: Another friend just said her baby’s paediatrician suggested breastmilk jaundice since no other cause could be determined. He even bluntly said it could be due to her “poor quality milk”!

My thoughts: I never knew that formula milk could be of “higher” quality than breastmilk. Excuse me, Dr., didn’t medical school teach you that breastmilk is nature’s PERFECT food for young homo sapiens?

Our Experience With Jaundice Handled By A Pro-Breastfeeding Paediatrician

Fortunately, our journey started right — we were recommended a pro-breastfeeding paediatrician. On Day 9, Vee became sleepy and looked yellowish. We brought him to the paediatrician for a check and his level was 354. The doctor advised hospitalisation for phototherapy.

We managed to grab a family room so that I could stay with Vee and keep breastfeeding him. The paediatrician was very encouraging and assured me that breastfeeding helps baby to pass lots of yellowish stool, which would eventually bring the jaundice level down.

The next morning, his level was down to 233, still high but phototherapy was no longer necessary. We went home, continued breastfeeding him and true to the doctor’s words, his jaundice went away after a few more weeks.

How to Avoid “Breastmilk”Jaundice Crap?

In my article “Breastfeeding: Top 8 Mistakes to Avoid“, mistake #7 includes lack of professional support. Misinformed advice by people of authority could break a budding breastfeeding relationship or burst a new Mum’s morale.

In Vee’s first year of life, we visited the paediatrician for tonnes of reasons (thanks to his super-anxious first-time parents) — jaundice, runny nose, swell near the anal opening, eczema, fever, etc. The doctor was always supportive of breastfeeding and found ways to help Vee get better without interrupting breastfeeding.

If you are determined to breastfeed, then seek out a paediatrician who is supportive of breastfeeding. If breastfeeding really needs to be interrupted, get advice from a qualified lactation consultant on maintaining milk supply and preventing nipple confusion. Your baby will thank you for this.

If you are a Mummy-to-be, what are your concerns regarding breastfeeding?
If you are a breastfeeding Mummy, how did you overcome the obstacles to breastfeeding in the early days?

—

Introduction

On my son’s first birthday recently, we also celebrated the first anniversary of our breastfeeding relationship. Breastfeeding provides numerous benefits – healthy for mother and baby, saves money, saves time, is convenient during travels and more. Despite these, many mothers end up supplementing their newborn babies’ diet with formula milk or completely give up breastfeeding in the early days post-partum.

From personal observation, below are the top 8 mistakes that prevent mothers from breastfeeding successfully.

Mistake #1: Not Well-Informed

Are you able to answer these:

What does the perfect latch-on look like?

What are a baby’s hunger cues?

How could you to prevent nipple confusion?

If you are clueless about the above, it is time to read up!

During pregnancy, attend a pre-natal class (preferably with your partner) conducted by a qualified lactation consultant. Learn all you can about breastfeeding and practise various breastfeeding positions. Talk to mothers who successfully breastfed and learn from their experiences. You may also join a breastfeeding support group.

Mistake #2: Not Latching On Directly and Pumping Instead

Breastfeeding baby directly provides the best stimulation for milk production, which is especially important in the first 4 to 6 weeks. During this period, it is not necessary to express the milk to see how much you are producing and determining if it is sufficient. The amount expressed is not indicative of the amount of milk that your body is able to produce.

My boy is able to suckle until the breasts are empty and if he needs more milk, he would continue suckling until milk is produced on the spot. The pump has never been able to do that for me.

Mistake #3: Not Feeding on Demand

Feeding on demand is the most important way to ensure that the milk supply is quickly established in the early days. Your newborn baby needs to be breastfed at least 8 to 12 times round the clock. Do not try to schedule her feeds or watch the clock when she is feeding. Simply latch her on once you notice her early hunger cues, and feed her until she is satisfied.

A newborn who is not crying hysterically finds it easier to learn latching on properly and wait for the let-down reflex. It is also easier to learn cradling her in the correct position.

Mistake #4: Buying a Tin of Formula Milk (Just in case)

Having a tin of formula milk on standby means you are more likely to use it upon facing obstacles or discouragement. People around you are also likely to ‘push’ you into supplementing when they see that your milk has yet to come in. Or worse, they could feed your baby supplement milk without your consent!

A full-term baby has body reserves to survive without any supplement in the first few days. In my case, my milk came in on day 4 and my boy survived well on colostrum before that.

Mistake #5: Feeding Water

A full-term newborn does not need water or any other drink unless medically-necessary. Foremilk (the first part of breastmilk) is thirst-quenching and much more nutritious than water. Water fills your newborn baby’s tiny tummy, she suckles at your breasts less and this leads to less stimulation for milk supply.

Mistake #6: Using Artificial Teats

Babies are very smart and know that it is much easier to get milk from a bottle teat than breastfeed directly. Feeding your newborn with expressed breastmilk (EBM) or water using a bottle teat could lead to nipple confusion and rejection of the real nipple. With less suckling at the breasts, there is less stimulation and less milk supply. If necessary, feed your newborn with a cup, spoon or syringe instead.

Mistake #7: Lack Family, Confinement and Professional Support

Delivery is a tiring process and it is important to have sufficient support to breastfeed successfully. You need a supportive partner who is well-informed about the do’s and don’ts of breastfeeding and help you fend off obstacles.

I personally find it important to also have a pro-breastfeeding obstetrician-gynaecologist, confinement helper, paediatrician and deliver in a pro-breastfeeding hospital.

Mistake #8: Lack Confidence and Determination

Without determination, you would find it hard to continue breastfeeding for long and waver easily when others doubt your ability to supply enough milk for your baby. My friends and I who have successfully breastfed stuck to the belief that we could produce sufficient milk and were determined to breastfeed our babies successfully, whatever it takes.

In short, focus on breastfeeding your newborn directly on demand (at least 8-12 times a day) round the clock, ignore discouragement, and seek advice from a lactation consultant if you need help. Enjoy the benefits of breastfeeding!

—

Summary: Breastfeeding Success Checklist

Read up and be well-informed

Latch on directly for the initial days and weeks, until supply is established

Feed on demand (at least 8-12 times a day)

Believe you can supply enough milk for baby (No need for formula milk as back-up)

Breastmilk contains lots of water plus nutrition. Breastfed babies don’t need water, unless medically necessary

When feeding supplementary or expressed milk, use spoon, syringe or cup to avoid nipple confusion

Get as much family, confinement and professional support as possible

Have confidence in yourself

When Baby V was five months old, he rejected the bottle teat. There was more than a month of unsuccessful attempts to feed him using a soft spout. Over one weekend in August, I chanced upon an article entitled “What to Feed the Baby When the Mother is Working Outside the Home” by Dr. Jack Newman, on the website of Breastfeeding Mothers’ Support Group (Singapore). It suggested feeding through an open cup and it suddenly occured to me to try using the Medela Disposable Baby Cup Feeder, which the hospital has given me when I delivered Baby V. (They are also available at a pack of ten at Robinsons, The Gardens.)

During lunch, I offered about 5ml of expressed breastmilk (EBM) to Baby V via the Medela cup. He sipped it, recognised it as his usual drink, finished the entire serving after several sips (some dribbled down from his lips) and wanted more!

What a simple and inexpensive solution! I should have saved the money spent on buying the cup with soft spout, which is essentially a bottle too.

Now I can sleep well knowing that someone else can take over feeding Baby V when the need arises.

Pros:

Feeding lip allows baby to drink milk without much spillage

Made of rather thick plastic — the cup can be reused even though its name says ‘disposable’

Cons:

Needs a caregiver to feed baby from the cup

No cover, so I store the cup in a mini Tupperware container

Over the past 6 to 7 months, I have been using a wide range of AVENT’s products — breastpump, washable breast pads, feeding bottles, steriliser and warmer. As the potential adverse effects of bisphenol A (BPA) on infants became more widely reported, I decided to switch Baby V’s feeding accessories to BPA-free ones.

(For more information on BPA, please refer to Wikipedia’s link here.)
(For more information on AVENT’s response to the BPA issue, please click here and go to FAQs.)

Although Baby V doesn’t drink milk from bottles, I occasionally freeze some milk to be subsequently warmed up for cup-feeding. The sterilisation and warming up processes (exposure to high temperature) could lead to BPA leach.

About two weeks ago, I finally saw BPA-free bottles at Robinsons and Isetan (The Gardens, Mid Valley). The new bottles are made from Polyether Sulphone (PES) — a BPA-free material with a natural honey-coloured tint, as stated on the packaging.

However, they were steeply priced relatively to the ordinary range:

260ml (9oz) bottles: 1 for RM49 or 2 for RM95

120ml (4oz) bottles: 1 for RM43

At Robinsons, the promotion was 20% discount and another 5% off by using Mastercard.
At Isetan, the discount was 10% rebate for CIMB cardholders on the last Sunday of the month.

Due to the high prices, I bought only two large bottles for immediate use with the breastpump. For storage purpose, I would certainly search for more cost-efficient bottles.

As for the many gently-used BPA-containing bottles that I have, they can still be used as water bottles (for cooled water) or recycled to store non-food items.

Pros:

Free from BPA

Work well with existing AVENT feeding accessories

Cons:

Highly-priced (lack bundled deals)

By the way, AVENT’s BPA-free manual breastpump is also available at Robinsons at RM289 (20% discount was not applicable).

Baby V has been exclusively breastfed since birth; as a Stay-At-Home-Mum, I enjoy the convenience of direct feeding. I gave him a bottle-feed 1-2 times a week so that he would get used to the bottle teat. When I observed that he LOVED the bottle teat, complacency set in and I stopped offering him expressed breastmilk (EBM) for a while (he was 3 months 3 weeks old).

A month later, I gave him a bottle-feed and he totally rejected it! Panic set in and I quickly searched for an alternative, which was when I bought AVENT Magic Cup Soft Spout.

For the past month, I have been offering Baby V about 10ml of EBM almost daily using the Magic Cup. He neither rejects the spout nor drinks from it; he simply bites it (maybe treating it as a huge teether) and plays with it using his tougue. Hopefully, he will learn to drink from the cup soon.

Pros:

Non-spill

Colourful design attracts baby’s attention

Cons:

The spout has a small part, which needs to be attached to it before use and detached for washing. This tiny piece can easily get lost during washing.

—

Update on 27 May 2010:

Vee is now 15 months old and drinks well from open cups. I was curious if he could now sip from this cup. If he does, I could use it as his water bottle. I let him try the cup, yet he still does not sip from it. Guess this cup is fated to belong to the next baby.

By 4a.m., I felt very uncomfortable and painful. A few hours later, I developed a 37.8 degree Celcius fever and felt chills all over even though I was wearing long sleeves and pants. It seemed like flu has set in, and I recognised these as symptoms of potential mastitis.

On my nurse’s advice, I placed cold cabbage leaves on the swollen area and rested in bed, feeding Baby V as frequently as possible. The cabbage leaves reduced the engorgement within half a day and I felt much better.

I also used a sterilised needed to burst a nipple bleb, which contains blocked milk.

Within a day, I recovered and continued to use cabbage leaves treatment for one more night. Fortunately, this problem was solved quickly. Otherwise, infection could have set in and I would have needed to seek medical help.

Pros (of cabbage leaves):

Simple and effective method of relieving engorgement

Low cost

Cons:

Leaves a vegetable smell behind for a while

During pregnancy, I bought six 36-piece boxes of Pigeon Disposable Breast Pads when they were on sale (2 boxes at RM36). However, I turned out to be a non-leaky breastfeeding mummy, so they were not very useful to me.

Since the breast pads were dry most of the time, I did not have to replace them often. In the end, the adhesive tapes frequently got stuck together at the second or third feeding session when I moved my nursing bra, causing much frustration.

I have since switched to washable breast pads, which are much more comfortable due to the absence of adhesive and cost-efficient. These disposable breast pads did prove useful when I was too ill and tired to handwash the cloth ones.

Pros:

Soft material

Relatively large for more coverage

Curved design fits breasts’ natural shape

Convenient when unable to do laundry

Cons:

Adhesive tapes could stick together when moving pad too much

Expensive (RM0.50/piece, which means at least RM1/day!)
